    Complete the form at medxpress.faa.gov/MedXpress/Disclaimer.aspx before your visit.
  • Government-Issued ID
    A valid driver’s license or passport.
  • Glasses or Contacts
    If you wear them, bring them with you.
  • Urine Sample
    You’ll be asked to provide one during your appointment.
  • First-Time Exam?
    A color vision test is required. (Additional fee may apply.)
